Mission Statement: to provide health and human services for people in need who are 60 years of age and older. To ensure the residents of Gregg, Harrison & Upshur counties, receive quality primary and behavioral health care.

Goals:
1) Increase the community’s awareness and understanding that mental health is essential to overall health.
2) Improve coordination between mental health and primary health care providers.
3) Develop an integrated system of medical and mental health care within the Longview Wellness Center clinic.

It is well known that there is a connection between the mind and body and that a patient’s behavior can dramatically affect his or her physical health as well as their response to treatment. The Wellness Bridge is an entirely new approach, a team approach to improving the quality of patient care. This approach integrates a Behavioral Health Car Specialist into the medical team. The team conducts a brief “solution focused” assessment and develops an individualized treatment plan with interventions. In this model the average number of visits would be between 3 and 4 sessions taking about 15 to 20 minutes each. These sessions might include problem-focused education classes, medication management, provider consultation, and/or brief individual instruction to address specific problems.
